If you aren't on Date-Check, Preferred 411, or Humaniplex (with network), I'll need you to furnish contact information for at least one provider who can vouch for you. Be sure the provider is willing to give you a reference before sending me her contact information (phone number, email address and website). Your information is completely safe with me, and I will never contact you at a time other than when you tell me I may call you. If you are the type of person who is unwilling to share your information, please save us both some time and don't contact me for an appointment. If you have been referred by an existing client of mine or another provider, please include their name in the message section of this form so I can thank them for introducing us.

Be aware that filling out this form does not mean you have an appointment. When we decide upon a place and time to meet, we will need to speak to confirm the booking. I don't accept an email as a confirmation. Confirmation consists of a verification of time, date and location. That's all. It occurs very discreetly and doesn't have to take more than a minute.
